INSURANCE TIPS

Please bring your most current insurance card when coming to our office to ensure that the insurance information we have on file (i.e. member id, group number, etc.) is correct at the time of service.  Your claim processing could be delayed and you could be held financially responsible if the insurance information on file is invalid.  Be sure to notify your employer/insurance company about any new additions to your family!  For questions related to billing and insurance, please call 727-323-2727.

PAYMENT

We collect your copay at check-in so that you may leave the office immediately following your appointment. This can help with your child’s comfort after vaccinations and saves you from having to wait around after your child’s visit.

 

Payment may be made with cash, check, Visa, Mastercard, or Discover and is required at the time services are rendered. Your copay is determined by your insurance card/company, which you should bring with you to each visit.

*Per the No Surprises Act (effective January 1, 2022) our office will provide uninsured or self-pay patients with a good faith estimate of services that will be billed.
